Privacy Policy

This policy explains what Longitude Care LLC collects, why, who we share it with, and the choices you have. It applies to longitude.care and the services offered through it.
Who we are, and who provides your care
Longitude Care LLC is a Tennessee company. We operate this website and the administrative side of the programs offered here — enrollment, scheduling, billing, coordination with pharmacies, and support.
We do not practice medicine and we do not prescribe. Clinical care is provided by independent, licensed clinicians and professional entities. Those clinicians decide whether care is appropriate for you, what to recommend, and what to prescribe. They are separate from Longitude Care LLC, they exercise their own professional judgment, and they hold your medical record.
That distinction matters for your privacy, because it determines which rules apply to which information. Under the federal health privacy law known as HIPAA, your treating clinicians are the covered entities responsible for your protected health information. Where we handle that information on their behalf, we act as their business associate under a written agreement that requires us to protect it and limits what we may do with it.
What we collect and how we use it
Information you give us
Name, date of birth, contact details, shipping address, and account credentials. Health information you enter into an intake form or share with us or your clinician — including your history, medications, allergies, symptoms, goals, and any photos or lab results you upload. Payment information, which is processed by our payment provider; we do not store full card numbers.
Information we collect automatically
Device and browser type, IP address, pages viewed, and how you arrived at the site. We use this to keep the site working, secure, and understandable.
Information from others
Your clinician, the pharmacy filling your prescription, and any laboratory you use may send us information needed to coordinate your care and your order — for example, that a prescription was approved, filled, or shipped.
How we use it
To create and manage your account. To pass your intake to a clinician for review. To coordinate prescriptions, labs, and shipping. To take payment and manage your plan. To provide support and respond to you. To keep the service safe and to detect fraud or misuse. To meet legal, regulatory, and recordkeeping obligations.
We will not use your health information to target advertising to you.
How we share information
We share what is necessary, with people who need it to do their part, and nothing more.
- Your clinicians and their professional entities — so they can evaluate you and provide care.
- Pharmacies — to fill and ship a prescription your clinician has written.
- Laboratories — where testing is part of your program.
- Service providers — payment processing, hosting, identity verification, communications, and analytics. They may use your information only to perform work for us, under contract.
- Legal and safety — when the law requires it, to respond to lawful requests, to enforce our terms, or to protect someone's health or safety.
- Business transfers — if the company is acquired or reorganized, information may transfer as part of that transaction, subject to this policy.
What we do not do
We do not sell your personal information, and we do not sell or share your health information for anyone else's advertising. We do not use your health information to build advertising profiles.
Health information and HIPAA
Your treating clinicians are responsible for the medical record they create about you, and they maintain their own Notice of Privacy Practices describing your rights in that record — including your right to see it, get a copy, request corrections, and get an accounting of certain disclosures. You receive that notice from them, and you can request a copy from them at any time.
Where Longitude Care LLC handles protected health information on a clinician's behalf, we do so as a business associate under a written agreement. That agreement requires us to safeguard the information, to use it only for the purposes it was given to us, and to report any breach.
Some information you give us — for example, an email address you enter to join a waitlist, before any clinical relationship exists — is not protected health information. We still protect it under this policy.
If you want to exercise a right in your medical record, contact your clinician. If you are not sure who to ask, contact us and we will point you to the right place.
Messages, cookies, security, and retention
Text messages
If you give us your mobile number and agree to receive texts, we may send you messages about your account, your order, appointment reminders, and — if you opt in separately — occasional updates about our services. Message frequency varies. Message and data rates may apply.
Reply STOP to any message to stop receiving texts. Reply HELP for help. Consent to receive marketing texts is not a condition of purchase or of receiving care. We do not share your mobile number with third parties for their own marketing, and mobile opt-in data is never sold.
You can unsubscribe from marketing email using the link in any marketing message. We will still send you messages about your account, your care, and your orders.
Cookies and analytics
We use cookies and similar technology to keep you signed in, remember preferences, and understand how the site is used. Most browsers let you refuse or delete cookies; parts of the site may not work correctly if you do.
How we protect information
We use encryption in transit and at rest, access controls that limit who can see what, audit logging, and vendor agreements that require the same. No system is perfectly secure, and we do not claim otherwise — but health information is held to the stricter standard throughout.
How long we keep it
We keep information for as long as your account is active and afterwards for as long as the law, our regulatory obligations, or legitimate business needs require. Medical records are retained by your clinicians according to the retention rules of their state and profession, which are often longer.
Your choices and rights
You can ask us to give you a copy of the personal information we hold about you, correct it if it is wrong, or delete it. You can ask us to stop sending marketing. Depending on where you live — including Tennessee, California, and other states with consumer privacy laws — you may also have the right to know what categories of information we collect and share, to opt out of certain uses, and to appeal a decision we make about your request. We will not treat you differently for exercising any of these rights.
Requests about your medical record go to your treating clinician, as described above. Requests about your Longitude Care account come to us.
Children
These services are for adults. We do not knowingly collect information from anyone under 18. If you believe a minor has given us information, contact us and we will delete it.
Changes to this policy
If we make a material change, we will post the updated policy here with a new effective date and, where appropriate, tell you directly.
