Longitude Care LLC

Terms of Service

Effective August 3, 2026

These terms are the agreement between you and Longitude Care LLC for your use of longitude.care and the programs offered through it. By creating an account, joining a waitlist, or enrolling in a program, you agree to them. If you do not agree, please do not use the service.

Who you are contracting with, and who provides your care

Longitude Care LLC is a Tennessee company. We operate this website and the administrative side of the programs offered here — enrollment, scheduling, billing, coordination with pharmacies, and support.

We do not practice medicine and we do not prescribe. Clinical care is provided by independent, licensed clinicians and professional entities who are solely responsible for their own clinical judgment and for the care they provide. Your relationship with a clinician is separate from your relationship with us, and nothing in these terms changes it or interferes with it.

These terms cover the platform and the administrative services. Your clinician's own consents, disclosures, and notices cover the clinical relationship.

Eligibility, your account, and what we ask of you

Who can use this service

You must be at least 18 years old, located in the United States, and in a state where the program you want is available. Not every program is available in every state, and availability can change with state law and with the pharmacies and clinicians we work with.

Your account

Keep your login details private and let us know promptly if you think someone else has used your account. You are responsible for activity that happens under it.

Information you give us

Give accurate and complete information, and keep it current. This matters more here than on an ordinary website: clinicians rely on what you tell them when deciding whether care is appropriate and what to recommend. Incomplete or inaccurate health information can lead to a wrong decision, and it can make care unsafe.

Do not use another person's identity, payment method, or health information.

Medical disclaimer

This service does not provide medical advice, and this website is not a substitute for care from a qualified clinician. Information on the site is general and educational. It is not a diagnosis, a treatment recommendation, or a promise of any result.

If this is an emergency

Do not use this service for emergencies. If you think you may be having a medical emergency, call 911 or go to the nearest emergency room. Do not wait for a reply from us or from a clinician. Our contact form and messaging are not monitored around the clock, and they are not a secure channel for health information.

A clinician decides — and may decline

Choosing a program or paying for a plan does not entitle you to any medication or treatment. An independent clinician reviews your information and decides whether care is appropriate, what to recommend, and whether to prescribe. They may decline, recommend something different from what you expected, or refer you elsewhere. That decision is theirs alone, and we do not influence it.

No guaranteed outcome

Individual results vary. Nothing here promises weight loss, symptom relief, a laboratory result, or any other outcome. Some treatments carry risks and side effects, which your clinician will discuss with you.

Plans, billing, and cancellation

Plans and pricing

The price, billing interval, and what is included are shown before you enrol and in the plan terms presented at checkout. Those plan terms are part of this agreement. Prices can change, and if they change for a plan you are already on, we will tell you before the change takes effect.

Recurring billing

Programs are subscriptions and renew automatically at the interval shown at checkout, using the payment method you provide, until you cancel. By enrolling you authorise those recurring charges.

Cancelling

You can cancel at any time from your account or by contacting us. Cancellation stops future billing. Details of when a cancellation takes effect, and what happens to a period already paid for or a medication already ordered, are set out in your plan terms.

Refunds

Refund eligibility is described in your plan terms. Medication that has already been dispensed and shipped generally cannot be returned or refunded, because federal and state law restricts the return of dispensed prescription drugs. Charges for services already provided — including a clinician's review, whether or not it results in a prescription — are generally not refundable.

Charges you should know about

Some fees may be charged separately from your plan, such as laboratory testing you choose outside an included panel, shipping, or replacement of a lost order. Anything of this kind is disclosed before you incur it.

Prescriptions, pharmacy, labs, and delivery

Prescriptions

If a clinician prescribes, the prescription is sent electronically to a licensed pharmacy in the United States. You may choose your own pharmacy where the program allows it. Pharmacies are independent businesses; they are responsible for dispensing, labelling, and the medication they supply.

Compounded medications

Some products offered through these programs are compounded — prepared by a licensed compounding pharmacy for an individual patient. Compounded drugs are not FDA-approved, are not reviewed by the FDA for safety or effectiveness, and are not the same as a commercially available brand-name product. Your clinician will discuss with you whether a compounded preparation is appropriate.

Laboratory testing

Where a program includes testing, orders are placed with licensed laboratories in the United States. Results go to your clinician and become part of the record they hold about you.

Shipping

Orders ship to the address you provide. Keep it current — some medications are temperature-sensitive and cannot be re-shipped if delivery fails. Risk of loss passes on delivery to the address you gave us.

Messages and reminders

By giving us your contact details you agree we may contact you about your account, your care, and your orders. Marketing messages are separate and optional — you can opt out at any time, and consent to marketing is never a condition of purchase or of receiving care. How we handle your information is described in our Privacy Policy.

Legal terms

Acceptable use

Do not misuse the service: no attempts to break, probe, or overload it; no scraping; no impersonation; no using it to obtain medication for anyone other than yourself; no reselling or diverting anything you receive. Misuse of prescription medication is unlawful and we will terminate accounts for it.

Our content

The site, its content, and the Longitude and Longitude.Care names and marks belong to us or our licensors. You may use the site for your own personal, non-commercial use. Content you submit remains yours; you give us permission to use it as needed to operate the service and provide your care.

Third parties

We work with independent clinicians, pharmacies, laboratories, payment processors, and other providers. We choose them carefully, but we are not responsible for their professional judgment or their independent acts and omissions.

Disclaimers and limits

Except where the law does not allow it, the platform is provided "as is," without warranties of any kind, and our total liability arising out of these terms or your use of the platform is limited to the amount you paid us in the twelve months before the claim. We are not liable for indirect or consequential losses.

Nothing in these terms limits any liability that cannot lawfully be limited — including liability for a clinician's professional negligence, which is a matter between you and that clinician and their own insurers, or any right you have under consumer protection law.

Suspension and termination

You can stop using the service at any time. We can suspend or close an account for breach of these terms, suspected fraud, misuse of medication, or where continuing would be unsafe or unlawful. If we do, we will tell you why where we are able to.

Governing law

These terms are governed by the laws of the State of Tennessee, without regard to its conflict-of-laws rules, and the state and federal courts located in Tennessee will have jurisdiction over any dispute.

Changes

We may update these terms. If a change is material, we will post the updated version here with a new effective date and, where appropriate, tell you directly before it takes effect. Continuing to use the service after that means you accept the change.
